Jan Nieuwenhuizen c8e87f3021 Switch fat-c/boot using environment variable BOOT.
* GNUmakefile: check for BOOT, set CFLAGS.
* mes.c: Remove hardcoded BOOT define.
* scripts/include.mes: include loop-0 when BOOT is set.
