core: use FIXED_PRIMITIVES rather than environment cache.
* cache.c: Remove. * define.c: Remove callers. * mes.c: Likewise. (vm_eval_env) [FIXED_PRIMITIVES]: Hardcode car, cdr, cons, null_p. * GNUmakefile: Update.
